You pay Google directly for your ad spend. I don't touch it and I don't take a commission on it. My fee is for the management: setting up or auditing your campaigns, structuring ad groups properly, writing and testing ad copy, managing negative keywords, and reviewing performance on an ongoing basis so spend isn't leaking to searches that were never going to convert.
